(1) An individual licensee shall immediately lodge a police report of any loss or destruction of an individual licence and shall then report to the Commission in respect of the loss or destruction.

(2) An application for a replacement of an individual licence shall be made in writing to the Commission and shall be accompanied by any information as required by the Commission and the fee prescribed for its replacement as set out in Table A of the First Schedule.

(3) If the Commission is satisfied that the loss or destruction of an individual licence was not the result of any fraud or gross negligence, the Commission may issue a replacement licence in the same form as the original but with the word "DUPLICATE" endorsed on the licence.
