(1) Every application for an individual licence shall be made in Form A as set out in the Third Schedule and accompanied by the prescribed application fee set out in Table A of the First Schedule.

(2) Notwithstanding subregulation (1), any person applying for an individual licence for the provision of a particular facility or service shall submit, wherever applicable, the following details:

(a) the information required under the Act and regulation 7; and

(b) such other information or document as the Commission may require in writing or pursuant to any guideline issued by the Commission.
