(1) Any person ("the appellant") who is aggrieved by any decision of the Controller under Part III of the Act may appeal by submitting a notice of appeal to the Minister within the time specified in subsection 18(1) of the Act and a copy of such notice shall be sent to the Controller.


(2) The notice of appeal shall state-

(a) the full name, address and other particulars of the appellant;

(b) the decision with which he is aggrieved;

(c) the grounds on which the appellant proposes to rely on for the appeal; and

(d) such other information or contain such documents as may be necessary for the purposes of the appeal.
